STARTING THE SEASON

All forms can be picked up at the informational meeting, the Middle School office, or High School Athletic Office.

1. Current Physical on File (click for blank file)

  • Needs to be updated every three (3) years.
  • Without a physical you CANNOT swim or dive.
  • If you do not know if you have a current physical on file, check with the Activities Director’s secretary in the main Senior High Office. (651-306-7001)

2. MSHSL (Minnesota State High School League) Form on file (click for blank file)

  • Updated yearly (7-12 grade only)

3. District #199 Form on file (click for blank file)

  • Updated yearly

4. Activity Fee Paid or Waived

  • Boys Middle School (6th-8th grade) $80
  • Boys Varsity (7-8th grade Varsity & 9-12 grade) $130

    5. Bring to Practice (it is recommended that you keep these in your locker at the pool)

    SWIMMERS-

    • Practice suits- swim trunks are fine (Speedos are NOT required)
    • Goggles
    • Personal water bottle (no glass bottles)
    • Towel(s)
    • Swim cap (optional)
    • Tennis Shoes & dry land training (varsity)

    DIVERS-

    • Practice suits- swim trunks are fine (Speedos are NOT required)
    • Personal water bottle (no glass bottles)
    • Towel(s)
    • Swim cap (optional)
    • Tennis Shoes & dry land training (varsity)
    • Chamois (Chammy)
